[ ] English-version of Taiwan Household + one (1) photocopy

[ ] In case the minor child is included in parent's Reacquisition/ Retention of Philippine Citizenship document - Bureau of Immigration/ MECO Reacquisition (RA9225) document + one (1) photocopy of the following: 

• Order;
• Order of Approval; and
• Oath of Allegiance;
