A. FOR OFW:

[ ] Personal Appearance of applicant
[ ] Current Philippine passport + one (1) photocopy of data page (page 2 and 3)
[ ] One (1) photocopy of passport pages 4 to 5 and 44
[ ] Valid Taiwan Alien Resident Certificate (ARC) + one (1) photocopy of the Front and Back Sides
[ ] One (1) photocopy of latest Taiwan Visa
[ ] Processing fee: TWD2,000.00
B. FOR DUAL/MULTIPLE PASSPORT HOLDERS:
[ ] Online Appointment: passport.gov.ph
[ ] Printed Application Form
[ ] Personal Appearance of Applicant
[ ] Bureau of Immigration/ MECO Reacquisition/ Retention of Philippine Citizenship (RA9225) document + one (1) photocopy of the following:
[ ] PSA Certificate of Live Birth or PSA Report of Birth + one (1) photocopy
[ ] PSA Certificate of Marriage/ Report of Marriage + one (1) photocopy
[ ] English-version of Taiwan Household + one (1) photocopy
[ ] Taiwan/ Foreign Passport + one (1) photocopy
[ ] Processing fee: TWD2,000.00
