Travel Document

 

A travel document serves as official identification for one-way, direct travel to the Philippines, particularly issued in the absence of a passport. It is provided to Filipino citizens abroad who urgently need to travel to the Philippines but are unable to the meet requirements for obtaining a regular passport.

 

TRAVEL DOCUMENT REQUIREMENTS

Filipino Citizens (Adult)

Personal appearance of the applicant

Duly accomplished Travel Document Form

Duly accomplished Affidavit Form

Two (2) pcs. of recent passport size photos (3.5cmx4.5cm) 

  - white background, colored, without eyeglasses nor earrings

Other documents such as PSA Birth Certificate, Philippines Government Issued IDs, Certification of Voluntary Departure (if applicable)

In case of lost passport:

Photocopy of the lost Philippine passport's data page (if available)

Copy of Report on the Passport Lost/Recovered issued by the Taiwan National Immigration Agency (NIA) 

  - must be in ENGLISH

*Original and Photocopy of Report of Birth and/or Affidavit of Admission of Paternity (if available)

*Original and Photocopy of Passport of Travel Document of the mother

*All original supporting documents will be returned

 

Fees:

Travel Document: NTD 900;

Affidavit of Loss: NTD 700 (OFW), NTD 1,000 (non-OFW),

Expedite: 200 each

 

The processing time is 3 working days including the submission date.
